“Let’s Go Golfing
With Dad” Fundraiser

Friday, January 12th 6:00 – 8:30 p.m.

This will be a fun filled evening for students and their fathers (Grandfather, Uncle or Special Person) to play 9 holes of miniature golf. The course can be seen at www.golfanytime.net and can be played more than once as time allows.

The evening will include:
  *  A challenging course set up in the gymnasium.
  *  Other golf games and lots of fun!!
  *  Two pieces of pizza and a soda will be available at wholesale prices.
  *  Extra pieces of pizza can also be purchased.

Cost will be $3.00 per person at the door. You will receive a putter, ball, scorecard and dinner ticket. Additional school age siblings are welcome to attend and will be charged $2.00 per player.

To make this event most successful, please fill out and detach the attendance form below and return it to school by Friday, January 5th. Please retain the above portion of this flier for event information purposes. No money at this time other than Hole Sponsor checks. Green Fee money will be taken at the door. If you would like to sponsor a hole the cost is $25.00.  The sign will be positioned on the tee and returned to you upon completion of the event.  Please indicate what you would like printed on the sign below.  Please make check payable to:  Hometown Middle School.  Please have your child return this form and check to the school office by:  January 5th.
